What’s New, Expanding or Relocating
Many of our expanded services are centralized in a 25,000 sq. ft. Community Health Center building at 1011 Rock Quarry Road. It is located right next door to the former Rock Quarry Road Family Medicine location.
Pediatric Medicine (relocated to 1011 Rock Quarry Road)
We provide medical care for children–babies to teens–at all of our locations. Our new Southeast Raleigh location offers both pediatric care and dental care for kids under one roof. We have board-certified pediatricians who believe in prevention and early intervention to keep kids healthy. We offer developmental screenings, parent education and connections to community resources.
Family Dental Center (relocated to 1011 Rock Quarry Road)
Our Dental Center serves children ages 0-19 and provides adult dental care to registered patients. Our Dental Center has relocated to our new Southeast Raleigh location at 1011 Rock Quarry Road. We encourage parents to bring their children to see a dentist from the first tooth, by age one.
Senior Health Center (now open at 1011 Rock Quarry Road)
Rex Healthcare and Advance Community Health have partnered to create a new home for the Rex Senior Health Center. The center provides enhanced care and expanded services for senior patients in Southeast Raleigh and beyond. Dr. Leroy Darkes is seeing patients at the updated Southeast Raleigh location.
Horizon Healthcare for the Homeless Program – expanding outreach
Our Horizon Healthcare for the Homeless Program provides primary care, substance abuse services, diabetes education, behavioral health and many other services to people experiencing homelessness. The program is anchored at our Southeast Raleigh location, but patients of all ages can be seen at every location. We also have an Outreach Center at the South Wilmington Men’s Shelter and the Women’s Center of Wake County. Men, women, and children are welcome. We have many partners in helping the homeless: the Wake County Continuum of Care Collaboration, the Wake County Homeless Working Group, the Wake County Housing and Homeless Coalition, and the North Carolina Coalition to End Homelessness.
